Submitting this form confirms that it is your intention to conduct a HolyBears, Inc. fundraising sale during the dates specified. You understand that all personal checks will be made payable to your organization and you in turn will make payment to HolyBears, Inc. You acknowledge that all sales are final, there are no returns and that defective products will be replaced at no charge; also that the order is to be pre-paid. You enter into this agreement in good faith and should your organization be unable to conduct the proposed fundraiser due to unforeseen circumstances, you agree to be responsible for returning all materials received within 30 days in good condition and pay all applicable freight charges that have been incurred. By submitting this form means you have read, understand and agree with the General Terms and Conditions listed above and have authorization to sign on behalf of your organization.
